Prepare to plan ahead
Before we can set up accommodations, we need documentation that helps us understand how to best support you. This information helps us understand your needs and develop a plan that supports your academic success. We’re here to help if you’re unsure what to submit.
What to bring
All documentation must be recent and signed by a licensed professional. We may ask for an update if your documentation is over five years old. If you don’t have formal documentation, we may ask you to complete VCC’s Disability Verification Form, which can be filled out by a medical professional.
Documentation must be from a qualified healthcare professional and should include:
- How your condition limits academic performance or participation
- Recommended academic accommodations or supports
Accepted documents include:
- Medical or psychological assessments
- Specialist or physician letters
- Psychoeducational assessments
Documentation tips
To make the process easier:
- Submit documents in PDF format where possible
- Use professional letterhead or ensure all contact information is included
- Make sure the diagnosis and impact on learning are clearly stated
